Scheuerman’s disease.

This concerns a growth disease of a vertebral body vertebra at the height of the top or bottom. Usually the thoracic level (back between the shoulder blades) is affected and 4 to 6 vertebrae are affected. Sometimes there are also central defects in the middle third of the strike plate which are called Schmorl impressions or Schmorl nodules.

The complaints typically occur at a young age. It concerns an interscapular (between shoulder blades) dorsalgia and dorsolumbalgia (transition from thoracic vertebrae to the low back) of a mechanical nature, ie usually increasing with load.

The radiographic examination points to a narrowed intervertebral space with an irregular outline of the anterior third of the affected vertebra. These phenomena are the result of insufficient bone formation.

The treatment is not surgical and is aimed at muscle strengthening and posture correction. If necessary, a specific brace for posture correction can be used.

A complication in Scheuermann’s disease is the development of a thoracic hyperkyphosis (strongly curved back) with shortening of the pectoralis muscles (chest muscles), psoas muscles (hip flexors) and hamstrings. (knee flexors).
As treatments, stretching of the pectoralis major and minor (chest muscles) may be prescribed in addition to gentle mobilization of the spine.

Muscle-strengthening exercises of the thoracic extensors and the adductor scapula (rhomboidei), ie the muscles that fix the shoulder blade, are indicated. All the shortened muscles can be stretched.

It is important to pay attention to posture correction during daily activities, which means that one must keep oneself upright, for example when working at a computer.

As a sport, swimming is recommended mainly breaststroke. Hockey, rowing and martial arts are contraindicated.

Scheuermann’s disease can also manifest itself at the level of the thoracolumbar junction (transition of the thoracic spine and lower back region).

In this case, a kyphosis is less pronounced and we rather find an expiration of the back. We speak of a flat-back (flattened back) phenomenon.

Treatment is again conservative and aimed at improving posture.

However, the results are less favorable compared to the thoracic form.
