A positive (or A+) is the phenotype or appearance. The genotype can be either DD or Dd, and this distinction is not made with a classical test. After all, it has no clinical significance.
Rhesus factors (C, D and E are the best known but there are a lot more) are proteins on the membrane of the red blood cells (they protrude a bit like antennae). To know if you are Dd or DD one has to investigate that further.
By the way, the same applies to blood group A: that can be AA or AO.
